Some patients complain about dark circles under their eyes, making them look tired. Another typical complaint is lower eyelid bags. Patients with deep tear trough deformities might feel they look older than their actual age. This is often due to the hollowed region just below the eye known as the tear trough, or nasojugal groove. This area extends from the inner corner of the eye, following a semicircular arc to the palpebromalar groove. Tear trough deformities can be a result of genetics or a true sign of aging. As we age, our fat pads fall or shift downward, creating a depression or hollowing of the skin. Other contributing factors are that collagen, elastin and hyaluronic acid are depleted during the aging process, which causes drooping and sagging of the skin. In addition, there are changes in the mid portion of our face that result in fat and volume depletion.
For some, the best treatment might be a surgical procedure called a lower blepharoplasty. However, many patients prefer a less invasive approach with less downtime and a less expensive alternative to surgery. Some patients opt for having fillers injected along the lateral and medial cheek, deep onto the periosteum. This helps lift the skin and mask the appearance of tear trough deformities by helping to pull the skin back to its natural position. Typically, these dermal fillers last 1-2 years. During the injection appointment, the area will be filled conservatively, with a follow-up appointment recommended for two weeks later.
The tear trough is a highly vascular region of the face. Bruising and swelling are possible post- injection.
